Restrictions: This program is a dental savings plan, not dental insurance and is only honored at this office, The Dental Boutique. It cannot be used at any other office, for referral to specialists, or for hospitalization/hospital charges of any kind. This plan cannot be used in conjunction with any other insurance plan of any kind. However, once dental insurance benefits have been maximized, this plan may be purchased to complete treatment. Membership is effective for 12 months from the day on which payment is received. Membership rates may be reviewed and adjusted on an annual basis with notification. Premiums are non-refundable. It is the sole responsibility of the member to maximize benefits by arranging the appropriate appointments within the 12 month membership period. If the appointments are not used, the member will not be entitled to a refund. Patient’s portion of bill is due at time of service.
